Two things...
What is published here was the first revision but they have since announced even newer policies based on feedback (mostly focused on things being ok in private or dedicated groups, but not in "public").
I believe that they are being enforced by a combination of mods and responding to complaints, but FetLife doesn't seem to be pulling an Instagram/Google move and looking through everyone's stuff to find things that violate the ToU/ToS. I base this opinion on not hearing people yelling about stuff like that happening.
